Information for patients

Dear Patient, you may examine 2 versions of the MDHAQ (multidimensional health assessment questionnaire), which many rheumatologists are using to track patients according to numerical data in addition to simple descriptions in an electronic medical record (EMR).

Two formats are available, either for a new patient or “return” patient.

Electronic versions are available through your rheumatologist if she/he has registered to collect secure, HIPAA-compliant, private electronic MDHAQs from patients.
