Myprivatemed.com is a computer program to calculate electronically-collected quantitative patient self-report MDHAQ (multidimensional health assessment questionnaire) scores for physical function, pain, overall clinical status, specific painful joints, symptom checklist, and 4 indices for overall patient status and to screen for anxiety, depression, and fibromyalgia.
